Perfect example of a basic and standard sliding genioplasty in a patient with no significant osseous excess or shortfall at the chin in no one of its axes; vertical height, horizontal width and forward volume were deemed optimal thus a multidimensional genioplasty was not indicated.

The technique planned was a standard horizontal sliding genioplasty without any special tweak at its osteotomy angle, aiming to forward the chin up to 10 mm without the so called jumping genioplasty or overlapping assembly, at approximately 45 degrees from the bisecting line of the chin so that the projection effect is entirely forward.
As a general rule, sliding genioplasties with osteotomies made at an angle of 45º from the bisecting line of the chin, that is horizontal, produce an effect of increase or reduction of chin projection exclusively at its anteroposterior or depth axis; osteotomy angles larger or wider than 45º from the bisecting line of the chin produce at the same time either effects of projection increase and vertical height reduction or projection reduction and vertical height increase, the larger the angle more the effect will be in the vertical axis rather than in the depth axis.
However, the patient suffered a remarkably sharp chin, with a boat keel shape; performing only a chin forwarding would entail a so called witch's chin due to the surfacing of the originally sunken osseous sharpness; to prevent this unsightly side effect it was planned performing an associated chin widening or horizontal chin augmentation of approximately 8 mm at its center in order to obtain a rounder shape, requiring the tailoring of a chip of bone graft, in this case harvested from patient's elbow or olecranon.
This means the ambitious forwarding of the chin was supplemented by a customized chin augmentation using a cubical bone graft.
Wiring with steel sutures allowed stable fixation without any issues.
