Inaccuracies and falsehoods in Cosmetic Surgery

We are aware of the enormous discrepancies of criteria that abound between different specialists, centres or companies, when concentrating on Cosmetic Surgery and each one of its treatments. As a medical activity in the Private Health Sector, the conflict of interests between the commercial world and the ethical problem does not always lie on the side of safety, professionalism or in the interests of the patient. This situation creates an enormous lack of confidence in the patient wishing to have Cosmetic Surgery treatment. Our philosophy is very clear. The patients are always first, their interests and their needs, in conjunction with the efficiency and safety of our treatments. To this end the medical facet of our activity is supreme. However, in contrast to our practice, many other centres or professionals offer information that is inexact, slanted, market oriented or even completely false and self interested, practising dangerous procedures that are ineffective or obsolete. To prevent unscrupulous actions we list a series of statements that, in our opinion, are inadequate, deceptive, or mere lies, that are frequently used at other centres to manipulate the wishes of the patient to a commercial and unethical end. Some of these are even popular among the very clients of these centres of Cosmetic Surgery, and become the regular thoughts and petitions of these same clients. Naturally, we completely rejects all the statements that can be read under the title of Inaccuracies and Falsehoods for every group of our treatments.

Is my case customary in secondary surgery?

Although the possibilities are numerous, it is true that certain situations are repeated more frequently among those cases received by our Specialists requiring repair. Among the problems resolved with us are the following:

-Encapsulated breast implants: cases of breast augmentation considered lost or irreparable, because they have been operated on once or more occasions in the past and appear hardened, deformed or painful.
-Submuscular implants: cases of breast implants inserted under the muscle that because of its poor characteristic result or a complication, need repairing.
-"Upward and downward movement implants": Cases of breast enlargement in which the technique has not been correctly executed and the implants move up and down with the arm movement or the contraction of the pectoral muscle, named as elevator breasts.
-Dislocated or malpositioned implants: Which are eccentric from the theoretical ideal center of the breast mound landmarked by the nipple, leading to grotesque look of the breasts.
-Misjudgement in implant selection: The width of the implants do not match the theroretical ideal size of the breasts base therefore produce lateral prominence or alternatively cleavage shortage of the implants.
-Renewal of implants: Because they are aged, broken, deficient from the beginning or because the volume is inadequate.
-Mastopexy: Insufficient, poor cosmetic result, or have suffered a complication.
-Sagging breasts: Cases of sagging breasts for which the treatment has only been a breast enlargement, inadequately performed and without having received specific treatment for the sagging.
-Breast reduction: Poor quality cosmetic result, insufficient elevation, slight volume or having suffered complications.
-Neck Lifting: Patients having received obsolete rejuvenation treatments, such as simply stretching the skin. Facial rejuvenation would have to be redone and also the neck would have to be treated which, in many cases does not receive the necessary dedication.
-Eyelids: Cases in which bags have not been completely removed or a cosmetic or even functional distortion has appeared.
-Rhinoplasty: Inadequate nasal corrections that are not very natural or are insufficient, causing breathing problems, sometimes due to poor cosmetic judgement, reconstructions in multioperated noses.
-Otoplasty: Shortage of approximation to the head, deformities, cartilage injuries, skin shortage.
-Liposuction: Poor performances that require removal of skin, technically deficient performances with under or over correction.
-Abdominoplasty: Spare skin, skin necrosis, insufficient muscular work.

Is there a treatment that removes all signs of facial aging?

No. Each sign in the face showing the passing of the years has a different origin. Certain problems derive from an excess of skin, muscular hypertrophy or excessive expression, solar aging, the effects of gravity on the deepest facial tissue, sagging of the muscles, progressive shrinkage of the face, accumulation of fat and aging stains to the skin, among others. It is a known fact within the international scientific community that facial cosmetic surgery consists of several treatments and, only with a profound knowledge of the aging process and by mastering all these techniques, can our Specialists offer the maximum degree of successful results.

Is it safe to have a rhinoplasty, a cheekbone or chin augmentation, a facial rejuvenation, a blepharoplasty or a lifting done using injected substances?

Injecting synthetic substances into the depths of facial tissue in order to enlarge or give volume to parts of the face is, in the opinion of world Specialists consensus, a totally inadvisable procedure. Solid implants are immeasurably better than injections, and the latter are not going to achieve the same perfection. Another disadvantage is that over time the injected substances tend to fragment, produce inflammatory hotspots or deformities, this is a disease named iatrogenic allogenosis. If the patient wishes, solid implants can be easily removed at any time, which is not the case with injected substances.

Can various facial rejuvenation techniques be combined in one operation?

This is quite normal and much requested by our patients. There is no reason not to perform an eyelid treatment together with an eyebrow lifting or a face and neck lifting, in the same operation, to give an example.

Do effective non-surgical complementary rejuvenation treatments exist?

Certain facial wrinkles are due to an excess of muscular contraction that can be treated with botulinum toxin injections, that is a highly effective and safe product. The wrinkles derived from solar aging such as the stains to the skin can be treated with superficial cutaneous techniques, such as laser or peeling. Those wrinkles that form around the lips, popularly known as crows feet can also be removed by deep laser or peeling procedure

Are filling products effective against aging?

No. It is obvious that filling products can only be applied where there is a lack of volume in a particular area. This procedure will not therefore eliminate wrinkles nor can it correct the effects of aging. In spite of this obvious reality many centres affirm that filling products help to give a younger look. Their use in these cases only leads to grotesque deformities that are, at times, irreparable. Do not be deceived and be advised by our Specialists.

Can the bags of fat or excess skin on the eyelids reappear?

The most normal evolution for patients who are operated on the eyelids is not to need further treatment in the future. If the patient is young, or their lifestyle or illnesses accelerate the aging process, then another operation may be needed, but not until many years after.

What is transconjunctival eyelid surgery or blepharoplasty?

Blepharoplasty or intervention to the lower eyelids can be performed in two ways. The traditional procedure is a visible cut to the skin just beneath the eyelashes that will leave a discreet but permanent scar. The most modern technique, which is recommended at an international level as the best and most beneficial to patients, is by gaining access to the lower bags of fat without cutting the skin by means of an approach through the lower conjunctival sac. The incision is made into the depths of the fold that exists halfway between the eyeball and the edge of the eyelashes. In spite of the advantages it is difficult to find Specialists like ours who can perform this technique, as it requires a high degree of qualification. W always offers you the best, and you can benefit from this technique that we regularly practise.

What type of anesthetic is most suitable in eyelids surgery?

A blepharoplasty operation can be performed with full guarantees using a local anesthetic. As it is a small surface area the volume of anesthetic substance used is minimal, making it a matter of choice. This is a treatment in which the Specialist works within the field of vision of the patient where unforeseen movements can cause the patient unnecessary emotional stress. Therefore our Specialists administer an associated sedative to relax the patient giving them an agreeable sensation of comfort, as well as allowing the Specialist to proceed with the treatment with ease. For your safety and in order to achieve the best results our Specialists recommend a local anesthetic with an associated sedative in a blepharoplasty operation, staying in the clinic for a few hours and being discharged the same day.

Can I have only the upper or the lower lids done?

Yes. On occasions the cosmetic problem is concentrated at one level, which can be treated without undertaking further unnecessary procedures.

Will there be a scar to the skin following the operation on the eyelids?

The techniques of a transconjunctival blepharoplasty operation to the lower eyelids consist in perfectly hidden incisions inside the eyelids and therefore completely invisible at a simple glance; as an alternative the external approach leaves a scar concealed under the eyelashes skin. Upper blepharoplasty operations are performed making incisions into the natural fold that forms on the eyelid when it is raised; the scars are therefore almost invisible and of a very high cosmetic quality. Can other procedures also be performed in the same eyelids operation?

This is a normal procedure and much requested by our patients. There is no reason not to perform an elevation of eyebrows or lifting in the same operation together with a correction of the eyelids, to give an example.

What means of approach is best to the lower eyelids?

Without doubt the best access is internal or transconjunctival, leaving no scars on the skin. Advantages include a quicker, less inflammatory and better recuperation. We do perform this technique as a regular basis with few exceptions on which we do not consider this treatment appropriate and we opt for open approach under the eyelashes. Our Specialists are among the few who master both techniques.

Is the healing really so swift in the eyelids?

The skin in the eyelid region is the area that heals the quickest in the whole body. In only a few days the first internal threads are eliminated. The inflammation lasts rather longer but only considerably so during the first few weeks.

Can the vision be improved?

Many patients suffer a reduction in their field of vision due to the excess of skin in the upper eyelid region because the folds limit the opening of these lids. The operation with us improves the function of the eyelids as well as the cosmetic aspect, as the excess skin is removed with precision.

What are the bags on the eyelids?

In the space between the eyeball and the bone that surrounds it there are some bags of fat that occupy this free space. With the passing of the years and aggravated by certain hereditary factors, or from those that have been acquired such as smoking or thyroid problems, these adipose packages grow causing hypertrophy. The increase in size of these bags makes them too big to occupy the original space and they begin to gain prominence, extending over the outer limits of the socket, making them very visible as bags under the skin. There are two bags under the upper eyelids, inner and middle and 3 bags under the lower eyelids, inner, middle and external. The treatment consists in eliminating the excess that overlaps the socket and does not affect the normal function of the whole at all.

Will my expression change if I have my eyelids surgery?

No. The rejuvenation treatments that we offer allow you to continue being the same person, with the same expression, just younger and with the same features, it is that simple and natural.

Will differences between each eyelid be corrected with a blepharoplasty operation?

A more or less remarkable difference in eyelids is normal in almost all persons and, not only does not need correcting but is unfeasible, to address the problem usually leads to inadequate results. If the assimetry is a complex one, some differences will always persist.

Is there any relation between the eyelid treatment and the eyeball?

No. Surgery on the eyelids only corrects the structures attached to the eyeball. The Specialist works at a superficial level in the space that exists between the eye socket and the eye itself, without affecting the eye at all.
