Is my case customary in secondary surgery?


Although the possibilities are numerous, it is true that certain situations are repeated more frequently among those cases received by our Specialists requiring repair. Among the problems resolved with us are the following:

-Encapsulated breast implants: cases of breast augmentation considered lost or irreparable, because they have been operated on once or more occasions in the past and appear hardened, deformed or painful.

-Breast reduction: Poor quality cosmetic result, insufficient elevation, slight volume or having suffered complications.
-Neck Lifting: Patients having received obsolete rejuvenation treatments, such as simply stretching the skin. Facial rejuvenation would have to be redone and also the neck would have to be treated which, in many cases does not receive the necessary dedication.
-Eyelids: Cases in which bags have not been completely removed or a cosmetic or even functional distortion has appeared.
-Rhinoplasty: Inadequate nasal corrections that are not very natural or are insufficient, causing breathing problems, sometimes due to poor cosmetic judgement, reconstructions in multioperated noses.
-Otoplasty: Shortage of approximation to the head, deformities, cartilage injuries, skin shortage.
-Liposuction: Poor performances that require removal of skin, technically deficient performances with under or over correction.
-Abdominoplasty: Spare skin, skin necrosis, insufficient muscular work.

What do your prices include?


Our prices include all the concepts planned by the Medical Team for the management of each surgery on the basis of the most common situation known by current state of the art medical knowledge: basic preoperative tests, implants, garments, surgical theatre, hospital stay, Medical Team fees, postoperative nursery and medical check-ups, V.A.T. (I.V.A. in Spanish) included. Any other additional charges are not included in our prices and must be paid by the client, like non-hospital and home medication and materials, local medical, surgical and nursery attention at the city of your residence, emergencies, complications and retouches, which are all plainly excluded. The pricing information is indicative only and does not represent a closed obligation with customers. Medicine and Surgery are not exact sciences, so the final price quotation for each case is not definitively set until the Specialist has evaluated the patient. Dimensions and Volume of breast implants


It is neither serious nor professional to use a bra size as a benchmark in the result of a breast enlargement operation. The size of the implant is neither calculated in units of volume (cc or ml) or weight (gr), which in itself, is not of great use. Much more important than the volume are the measurements. Implants have a base or diameter and a height or projection at the peak. In reference to these measurements, implants can have a low, medium, high or very high profile that respectively mean that for equal volume they have more diameter and less projection and so on until they can be of a very narrow diameter and very high projection. Choice should be selected in relation to the breadth required by the patient's breast that then determines the diameter of the implant. The projection can then be established in accordance with the wishes of the woman and the particularities of each breast (volume, excess of skin etc.) The same implant could be suitable for one woman but incorrect for another, so that size cannot be compared between two cases. It must be considered that the final result is the sum of the volume and shape of the patient's breast plus those of the implant. Can other procedures also be performed in the same liposuction operation?

Is there a treatment that removes all signs of facial aging?


No. Each sign in the face showing the passing of the years has a different origin. Do breast implants correct sagging or empty breast?

Is it safe to have a rhinoplasty, a cheekbone or chin augmentation, a facial rejuvenation, a blepharoplasty or a lifting done using injected substances?

Is it true that I have to massage my breasts after implants enlargement?

Are all noses operated on the same?

Complications of breast implants

Will any scar be noticeable on the skin following the nose operation?

Can the abdomen problems reoccur after an abdominoplasty?


Regions treated in an abdominoplasty do not reproduce the same problems unless the cause of the problem is repeated in the form of hypertrophy of the abdomen.

Will the laterality and assimetry of my nose be corrected with a rhinoplasty?


A more or less remarkable difference in both sides of the nose is normal in almost all person and, not only does not need correcting but is unfeasible, to address the problem usually leads to inadequate results. If the laterality is postraumatic or postsurgical the Specialists can improve the nose with other nasal treatment techniques. If the assimetry and laterality are complex ones, some differences and laterality will always persist.

Does the sensitivity of the nipple and areola change after breast enlargement ?


Either with areolar or submammary fold surgical approaches to insert the implants it is possible to retain maximum sensitivity in the areola and nipple. Temporarily an enlargement in sensitivity in the area can be felt or, more infrequently, a reduction in sensitivity, which returns to normal after few weeks or months. It is extremely uncommon that sensitivity remains impaired as a long-term complication.

Can I have an operation if I have a broken nose?


One of the described techniques is the post-traumatic rhinoplasty and septoplasty, designed for patients whose noses have been considerably deformed in an accident or also with airway problems. Our team of Specialists is able to effectively resolve this situation.

Can a mammogram be taken if I have breast implants?


When it is necessary for you to have a mammogram and other imaging studies you only have to advise your doctor and the technician performing the test that you have implants. The reliability of the test is the same for women who have been operated on. Mammograms are not necessary as a regular control of the breast implant.

Can the extracted fat reproduce itself if I gain weight after a liposuction?


The extracted tissue does not reproduce itself. In areas of lipodystrophy or chronic presence of fat a new accumulation is not to be expected, even if weight is gained following the treatment. If the patient gains weight following a liposuction operation the volume will appear in non treated areas, particularly those where calorific deposits usually accumulate.

Will there be any scar to the skin following the eyebrows, face and neck rejuvenation operations?

Do effective non-surgical complementary rejuvenation treatments exist?

Are filling products effective against aging?


No. It is obvious that filling products can only be applied where there is a lack of volume in a particular area. This procedure will not therefore eliminate wrinkles nor can it correct the effects of aging. In spite of this obvious reality many centres affirm that filling products help to give a younger look. Their use in these cases only leads to grotesque deformities that are, at times, irreparable. Do not be deceived and be advised by our Specialists.

Can pregnancy stretch marks be removed with an abdominoplasty?

Can the bags of fat or excess skin on the eyelids reappear?


The most normal evolution for patients who are operated on the eyelids is not to need further treatment in the future. If the patient is young, or their lifestyle or illnesses accelerate the aging process, then another operation may be needed, but not until many years after.

What difference is there between the double chin and an aged neck?


Our patients frequently request a correction to the double chin when the symptoms presented are of an aging neck. The double chin shows an accumulation of fat just beneath the chin, usually hereditary and appearing in young patients. However, aging neck presents a combination of excess skin, muscular flaccidity and a general accumulation of fat in the neck. The recommended treatment for the double chin is liposuction and, for an aging neck, a modern muscular lifting is appropriate.

Will any visible scars remain following the abdominoplasty operation?


The technique involved in an abdominoplasty consists in discreet incisions in the upper pubic fold, at approximately underwear level. These incisions are extended laterally sufficiently into the hips to remove the excess skin. On occasions it is necessary to make a hidden incision into the tummy button. There are several reasons for rejecting local and epidural anesthesia. Firstly, because an epidural anesthetic applied at a higher level than the tummy button implies considerable respiratory risks. The need to apply a local anesthetic in the upper abdomen area as a complement to the epidural anesthetic is a risky practice, both painful and unjustifiable. For these reasons the anesthetist is often obliged to improvise by administering an uncontrolled and highly dangerous general anesthetic. Finally, we reject this practice because the sensation block out is short term and not always total, causing pain to the patient and not allowing the surgeon to perform such a meticulous and precise treatment. For your safety and for the best results, our Specialists recommend a general anesthetic for an abdominoplasty, with at least an overnight stay in the clinic. Can I become pregnant again following an abdominoplasty?


There is no reason why a woman having undergone this operation should not go through another pregnancy. It is recommended that a period of 12 months elapse, depending on the technique performed, before becoming pregnant again. During that time the use of contraceptives is recommended.

Can I remove cellulitis with liposuction?


No. Neither liposuction nor any other technique or surgical treatment removes cellulitis and whoever states the contrary, is literally deceiving the patient. Cellulitis is a chronic irregularity of the circulation within the fatty tissue, generally in the legs and on the buttocks, that creates an oedema or accumulation of liquid within it. Cellulitis can appear in chronic accumulations of fat or lipodystrophy. From the outside an increase in volume and thickening of the contour is visible together with the skin irregularities known as the orange peel effect. Treatment for cellulitis is never an operation. Other treatments exist such as lymph drainage or others that control the problem and improve it, although it is never completely removed, or for very long, so the treatments have to be repeated.

Will there be a scar on the skin after the liposuction operation?


Liposuction techniques consist in tiny incisions, of merely a few millimetres that, after a while become almost imperceptible. How long should I wait between two surgical procedures?

What attitude should I take if my result is unsatisfactory?

What volume will remain following the removal of the breast in males?


The ribcage of a patient remains in total harmony in any operation performed by us. How long do breast implants last?


It is not possible to ascertain with certainty the foreseeable duration of breast implants because the available statistics refer to a wide range of factors including different brands, positioning factors, techniques involved, the patient biologically herself etc. Besides, the manufacturing of the product periodically improves. The degradation of breast implants is mostly biological and not mechanical, since the device suffers disintegration at the end of its lifetime due to the immune cells of the surrounding tissues in a slow and progressive period of years. Currently we are changing implants that have been inserted in the patient for between 10 to 20 years. It can even happen that a woman may have a torn implant and is not aware of the situation for years either, in the shape or through symptoms. It can be said that breast implants are not lifetime devices and will need replacement throughout woman's life, without this supposing any kind of health risk to the patient even in case of undiagnosed rupture. Can a mammogram be taken after having breast elevation or reduction?


When it is necessary to have a mammogram and other imaging studies you only have to advise your doctor and the technician carrying out the procedure of your breast elevation or reduction operation. The reliability of the test is the same for women who have been operated on. Mammograms are not necessary as a regular control of the breast elevation or reduction.

What are tubular or tuberous breasts and how do they influence breast elevation?


This term is applied to an anomaly present in certain patients whose breasts are not only small but also have a special structure. It refers to breasts constrained by a fibrous covering, giving the breast a conical shape, with excessive elevation of the lower furrow, a large areola and with a ruptured aspect, having insufficient skin in the medial area of the breast. - Subfascial: The fascia (thin fibrous shell) that covers the pectoral muscle is lifted up to be used as wrapping structure for the breast implants. This technique is considered optimal in current surgical state-of-the art since allows suitable prevention of capsular contracture (hardening, displacement and visibility of the implant, sometimes disturbances or pain), avoids dynamic mammary distortion and reduces the likelihood of other complications, easing the recovery period to a short, much safe and almost pain-free stage.

- Subglandular: Despite being a technique with low aggressivity it is inadvisable due to high rate of capsular contracture (hardening, displacement and visibility of the implant, sometimes disturbances or pain) and other issues with the breast implants.

- Intraglandular: Strictly proscribed in practical use since the weight of the breast implants lies on the breast skin and gland therefore producing breast ptosis (drooping).

Must the breast implants be checked up?


Between 6 and 12 months after the operation the patient will be medically discharged. From that time on the patient does not need to undergo routine checks. Underwired bras can be used on occasion but are not suitable for daily use. If the patient feels like it she can go braless.

What size shall I be after breast enlargement?


A brassiere is neither a valid nor reliable measurement. Bra sizes are indicated by means of a number in relation to the outline of the lower part of the breast, followed by a letter (A, B, C,&) that is the cup size. The outline of the lower part of the breast should not be substantially transformed following the operation, but the cup should increase. In any case a woman can use two different bra sizes and both can be perfectly suitable, so that this form of measurement is not going to help us to evaluate the volume of breast tissue in the final result.

Can I travel alone to your facilities or I'd rather get my treatment done accompanied with someone?

What are the differences between spherical or round and anatomical or teardrop profiles of breast implants?

These prostheses replicate the original structure of a natural breast, being round shaped at their lower half and differentially wedge shaped at the upper part of the breast and the cleavage; their main advantages are their more natural results at the upper part of the breast and the fullness in filling that mammary upper pole and cleavage. They are breast implants for patients seeking the most optimal results, especially those cases in need of full breast shape replacement (very small or hypoplastic breasts) or with specific shortage of breast tissue at the upper mammary pole.

What are low, medium, moderate, high and extra high profiles of breast implants?


Breast Implants Profile Height
In order to simplify concepts can be said that there are two profile height groups of breast implants, the low profile, also called flat shaped, and other with high profile, also called prominent shaped. With both models patients can achieve excellent results.
- This is not an absolute value in breast implants; it is rather a ratio between profile projection length (distance from the prosthesis rear side and the maximum projection point or peak of the implant) and width of the prosthesis base.
- This means that, with the same base width, low profile implants projection is shorter (lower) than those of high profile; with such same base width high profile implants have longer projection (higher).
- With equal base width low profile implants provide less projection and less breast size than high profile implants; with such same base width patients obtain with high profile implants larger breast size and longer projection than with low profile implants. - Low profile implants are therefore, in equal volume, wider and flatter than those of high profile; high profile implants, under equal volume comparison, are narrower and more prominent than low profile ones. - Two implants may have the same base width and be one of low profile and the other high profile, and in turn two implants may be the same projection length and have low and high profiles respectively. - Talking about low profile implants equates to discuss about broad-based implants; and talking about high profile implants equals to mention narrow base implants. - This explains the null importance of nominal volume (cc or ml) or weight (gr) of the implants at the time of their selection, since these parameters only indicate the amount of liquid or mass that can be inside the prosthesis, which give no indication of their suitability for individual patients, they would simply be nonspecific measures of gross amount. The critical selection of the ideal implant for each patient should be made on the basis of their dimensions (base width, profile height), not by weight or by volume. - There exist numerous intermediate profile height ratios between the lowest and the highest, with different names depending on the manufacturer concerned (flat, low, moderate, intermediate, high, extra-high, etc.), but always keeping this concept about the relationship between projection height and base with. - The lower profiles are best suited for situations where the implant will have on top a significant amount of glandular tissue (women with medium or large breasts) or due technical reasons a lot of tension is not desirable in the tissue, and so the implant must make room in the projection for the set to fit perfectly. They are also suitable for women with large breast width, which therefore need quite wide implants, and not seeking a too large breast size. - The higher profiles are preferable in cases which the implant will have on top small amounts of breast tissue (very small or hypoplastic breasts) and the prosthesis is aimed to replace the majority of missing breast volume. They are also adequate for women with small breast width, which therefore need quite narrow implants, and seeking a quite large breast size.

What type of anesthetic is most suitable in breast removal in males?


Without doubt we recommend the safe techniques of a general or total anesthetic. Are the breast implants placed beneath or above the muscle?


Traditionally and in the early years of breast enlargement the implants were placed between breast and pectoralis muscle, this was named the subglandular positioning of breast implants, however with traditional breast implants this technique produced high rate of hardening or capsular contracture also named encapsulation, artificial outcomes and other complications Nowadays and with modern implants the Specialists have two safer options to locate them that offer low rates of capsular contracture and both equally brilliant in their cosmetic results: under the pectoralis muscle named as submuscular, and under the pectoralis muscle fascia named as subfascial.

What treatments do you consider inadvisable?


For different reasons we do not perform certain treatments considering that they do not fulfil the internal regulations and philosophy regarding quality and safety we like to offer our patients. -Calf enlargement with normal or hypertrofic muscles, we only recommend as a consequence of nervous paralysis, postrauma atrophy, degenerative or congenital conditions, as this is a muscular contraction region.
-Endoscopic facial lifting, as the result is worse and not as long lasting as techniques without an endoscope, these latter techniques having been scrupulously tested and the incisions are minute.
-Facial lipofilling or filling. Nowadays it is known that this treatment only lasts a few months and also creates a grotesque appearance to the face.
-Facial filling with permanent or long lasting synthetic materials, that with the passing of the years, has a migratory or problematic effect or, in some cases, the consequences to date are not even known.
-Ultrasonic liposuction, that shows no advantages in practice in relation to traditional liposuction. It is also very dangerous sometimes producing irreversible consequences.
-Liposuction in unsuitable body regions such as arms, breasts, buttocks, anterior thighs, ankles and calves.
-And all untested techniques, including those that are unforeseen, unsafe, insufficient or excessively aggressive.

Can the cheekbone and chin implants break?


The top quality facial implants used by our Specialists have been subjected to so many rigorous quality controls that premature breakage is non-existent in practice. Which techniques are the best alternatives for a cheekbone and chin augmentation, implants or osseous surgery?


In the case of chin augmentation we have an excellent option which is the osseous genioplasty. It is the advancement of a portion of the bony chin, which is slided forward to achieve the wished profile and a real volume increase, without the need for implants. Its main advantages are that we are using patient's own tissues to obtain the effect, the result is absolutely natural and that it lasts forever. What chances of success does a repeat operation in secondary surgery have?

Are there any risks with the anesthesia?


Without doubt the safest anesthetic technique in healthy people and in the majority of operations in Cosmetic Surgery is a general anesthetic. This technique provides the highest degree of control over the situation regarding the patient.

A false sentiment exists in many patients that local anesthetic is safer than general anesthetic. In general, this opinion is incorrect, for local anesthetic is high risk in surgery that affects large areas or determined parts of the anatomy.

Local anesthetic substances are not innocuous, and over and above certain doses, which may be necessary in extensive areas of treatment, can produce serious and irreversible complications. Furthermore, application on a large area can only be achieved by causing a lot of pain to the patient and a complete blocking out of sensibility can never be obtained. This requires additional anesthetic resources to be applied with scant control through not having been correctly planned.

In the same way, many patients may believe that if their treatment requires an overnight or long stay in the hospital it must be safer or they are receiving better attentionless aggressive. This is one of the most dangerous deceptions a person can fall victim to at other centres.

Only at a hospital can the care, medication and vigilance required in the immediate postoperative phase be provided in order to avoid complications or find the correct solution. All forms of anesthesia may be safe or dangerous, dependent on the appropriate application being given in the correct instance. But also it is a well known scientific fact that early discharge from hospital and the sooner the patients leaves the sanitary facilities the rate of infections and other complications is dramatically reduced.

We follow the principle of establishing maximum guarantees using the less invasive and less hazardous postoperative management, selecting the combination that offers most safety to our patients, only taking into consideration medical factors.

What type of anesthetic is most suitable in cheekbone and chin surgery?


A cheek or chin implant operation can be performed with full guarantees using a local anesthetic. As it is a small surface area the volume of anesthetic substance used is minimal, making it a matter of choice. This is a treatment in which the Specialist works within the field of vision of the patient where unforeseen movements can cause the patient unnecessary emotional stress. Therefore our Specialists administer an associated sedative to relax the patient giving them an agreeable sensation of comfort, as well as allowing the Specialist to proceed with the treatment with ease. For your safety and in order to achieve the best results our Specialists recommend a local anesthetic with an associated sedative in a cheek or chin operation, staying in the clinic for a few hours and being discharged the same day. Will the ears stick out again?


The characteristics of the most efficient techniques of an otoplasty operation consist in producing a permanent modification of the outer ear, maintaining the shape desired by the patient. Reproduction of the separation is quite an undesirable complication.

High, medium and low cohesivity breast implants, which one is best?


From a starting point let's state that all the breast implants available in Spain and other European countries fulfill the standards according to the European Community legislation that requires them to be cohesive, and therefore they offer maximum guarantees with little or no quality difference among brands and manufacturers, however cohesivity or density of the inner silicone gel is a key factor to be focused on. The higher the density of the inner gel is, the less likelihood to produce visible waves named rippling, the longest duration of the implant and the safest in case of rupture, always keeping maximum naturality in the touch feeling to the operated breasts. That is the reason why different degrees of cohesivity or gel density do exist. High cohesivity gel implants are more modern and most recommended, since much better performance is obtained with them rather than with medium or low cohesivity breast implants. In the theoretical situation of a rupture or leakage of the implant, they all may leak small amounts of silicone that themselves are not a health threaten thought are not desirable, high cohesivity implants as well as in other models but in significatively lesser amount than in low or medium cohesivity ones. Leakage of silicone means no risk for the patient (unless the implant remains unchanged for many years, which is practically impossible), as silicone is already in contact with patient's tissue at the outer shell of the implant, being a product perfectly tested and safe. This situation remains unchanged in the case of leakage from inside of the implant, as it is silicone as well as the outer shell, being that leakage confined in the scarry or fibrotic shell grown around the prosthesis which is named the capsule and thus not spreading to the rest of the organism. Medium or low cohesivity implants are not risky or unsafe implants, however highly cohesive silicone gel filled implants offer the highest guarantee for patient's health.

How long in advance is it necessary to book my operation?

The preference of the majority of Specialists and also of the patients is through the areola or periareolar. The incision is made on the edge of the darker skin of the areola where it is joined with the normal skin, or in omega shape through the areola and not touching the nipple if the areola is pale or pinky so that it is perfectly disguised. Can other procedures also be performed in the same chin and cheekbone implants operation?

What type of anesthetic is most suitable in calf implants?

What bra is most suitable after breast elevation or reduction?


The first few weeks following the operation it is recommended that a cupped, support bra made of elastic or cotton that feels comfortable be used. Once fully recuperated the patient can use any bra she feels to be comfortable providing no marks are left on the skin.

What happens with the passing of the years after breast elevation or reduction?


Aging of the breasts is inevitable with the passing of the years and is manifested in the sagging of the breasts. This process affects the original tissue most of all, and the implants scarcely at all. The sagging of the breasts can be considered minimal for many years.

What size will I have after breast elevation or reduction?


Bra size is neither a valid nor reliable measurement. Bra sizes are denoted using a number for the lower outline of the breast, followed by a letter (A, B, C&) indicating the cup. The size of the lower outline of the breast should not be substantially modified following the operation but there will be a change in cup size. In any case a woman can use 2 different sized bras and both can be comfortable, so that this instrument as a measurement is not going to help us evaluate the determined volume in the final result.

What tests will be carried out before the anesthetic?


As a general rule all our patients are examined prior to the anesthetic and given a series of preoperative tests. What are the differences between saline solution filled and silicone gel filled breast implants?


Modern breast implants can be filled with silicone gel or with saline solution (body water).

- Saline Filled: Their main advantage is the biocompatibility of the filler in case of leakage, rupture or at the end of the prosthesis lifetime, since the serum filler is a fluid normally present in human body and therefore naturally resorbed with no issues; also the replacement procedure is simpler since the surgeon has to do little surgical toilet. Their cons are the possibility of early deflation and their likelihood to produce rippling (visible and palpable wrinkling), coldness in the winter, bubbling (audible or felt) and water balloon touch feeling; all the former more probable in slim or skinny patients. They are breast implants for patients who really wish to say no to silicone.

- Silicone Gel Filled: Silicone gel molecules may have different degrees of bonding between them; this is the so-called degree of cohesivity. Ancient breast implants models were filled with non-cohesive fluid silicone and this lead to different complications, specially rippling (visible and palpable wrinkling), leakage through the shell and dissemination of the silicone in case of rupture of the implant or at the end of its lifetime. Modern breast implants are filled with cohesive silicone gel in a variable degree of cohesivity: low, medium, high and ultrahigh. The higher the cohesivity the lesser the probabilities for the previously mentioned complications to appear. Silicone gel filled breast implants eradicate, prevent or reduce the complications traditionally associated with saline filled implants, especially early deflation and rippling (visible and palpable wrinkling), producing natural touch feeling. Their main disadvantage comes at the end of the prosthesis lifetime, requiring a deeper surgical toilet though this is a safe routine procedure. They are breast implants for patients seeking the most optimal results, especially those models with ultrahigh cohesivity.

Is there any relation between the duration of an operation and its safety?

Structure and composition of breast implants


Breast implants are composed of an external covering and an internal content. In all models the outer covering is made of silicone, and it is the area where manufacturers have striven to improve the duration and quality of their product.

The main differences in terms of composition can be found in the interior; these can be made of silicone gel or saline serum (other substances such as hydrogel or soya oil have been withdrawn from the market). Implants filled with silicone appeared initially and, even after many years of experience, continue to be the ones that offer the best results and greater safety.

In spite of all the controversies arising years ago, most of them unfounded, silicone gel implants continue to be the best and no other substance has managed to supplant them. We recommend silicone filled cohesive gel implants.

Serum implants are not recommended by our Specialist because they are colder in winter, they sometimes produce liquid sounds, the breast shows folds called rippling, the touch is artificial and, in general, they empty prematurely thereby offering a short duration, particularly if we compare them with the silicone implants. It must be remembered that even saline serum implants have an outer covering made of silicone, which is the part that is in contact with the patient's tissue.

Silicone is a substance widely used in medicine and surgery, and forms the manufacturing base of various kinds of implants (ophthalmology, vascular, orthopaedics, breasts, buttocks etc.) It is perfectly biocompatible and allergy to silicone is practically nonexistent. There is no rejectio of saline.

Silicone has no relation to immune illnesses or those of the breast and neither hastens nor prevents cancer of the breast. It is perfectly compatible with breast-feeding and should the covering tear, it does not dissolve into the maternal milk and therefore is not passed on to the baby. Examination by mammogram is perfectly viable with the same degree of reliability.

Certain implants exist with an especially dense and coheseive silicone interior; these are called cohesive gel implants. These are recommended by our Specialists because they are safer and reduce the likelihood of visible waves on the breast called rippling.

Is there an age limit to having an operation?


In general, there is no established age limit. The state of health is more important than the age and, depending on the treatment, age is the factor making the operation suitable. It would finally be the evaluation of numerous medical and technical factors that would determine the best kind of operation for each case.

How are your anesthesia protocols?


We are aware that our work consists in offering safety to patients requiring Cosmetic Surgery at a top level. We therefore totally reject deceitful, dishonest, unsafe and painful anesthetic practices.

It may seem paradoxical or contrary to habitual practice, but local anesthetic implies an enormous risk in certain operations, such as major facial surgery, breast and abdomen surgery or in large liposuctions, and, in spite of this risk, local anesthetic is used in many centres with little professionalism and excessive commercialism.

We have established a protocol that offers the highest levels of safety and comfort in each treatment, which we then adapt to every particular case if necessary, evaluating exclusively queries of a medical nature:
-General Anesthetic: Rejuvenation of face and neck, full nose rhinoplasty, osseous genioplasty, breast surgery, extensive liposuction, abdominoplasty or tummy tuck, in general for any kind of major and combined surgery.
-Local Anesthetic with associated Sedation: eyelids, ears, cheek and chin implants, tip of the nose, small liposuctions, small retouches, in general for any kind of minor and simple surgery.

Should I avoid the sun after the operation?


No. Once the initial postoperative period has been completed the patient can live a normal life. Our Specialists only recommend the use of a high factor protective sun cream on incision areas for the months following the operation. This precaution will help to avoid permanent pigmentation and should be applied during all seasons of the year as long as the scars look red or pink.

Types of Anesthesia


Different anesthetic procedures are chosen with a view to applying the appropriate technique to each operation, always bearing in mind the safety and recuperation of the patient.

-General or total Anesthesia: This is the most traditional kind of anesthetic, in which the patient is completely ""put under"". It is considered to be the safest and most comfortable in the majority of Cosmetic Surgery treatments. The main advantages are that the patient has only a memory of having ""gone to sleep"" for a few minutes, irrespective of the time spent in the operating theatre, and also that the surgeon can work with absolute concentration without a time limit. This method requires at least a day case stay in the clinic.

-Epidural Anesthesia: This is the same technique that is usually applied in childbirth. It consists in applying a small amount of local anesthetic around the nerve ends of the lumbar region in the back. In this way all sensation is completely blocked out from the level of the tummy button downwards, through the lower abdomen and down the legs. A light sedative is given in conjunction with this method in order to prevent anxiety and stress in the patient. The advantage is that it can be used as a substitute for a general anesthetic in certain operations such as on abdomen or on legs, when that general anesthetic is disregarded. A day case stay in the clinic is recommended.

-Local Anesthesia: This consists in injecting anesthetic agents into the tissue area to be operated on, around the area or by blocking out peripheral nerves that provide sensibility. Depending on the treatment a light sedative is also given to the patient to prevent anxiety and stress. The main advantage is that a stay in the clinic is not necessary. It is appropriate for operations on small areas that do not require a high volume anesthetic agent.

-Sedation: This consists in administering tranquillising substances to patients who will remain awake throughout, albeit in an agreeable relaxed state which also produces amnesia so that the patient only retains a vague and agreeable memory of their stay in the operating theatre. Sedation is used as a complementary technique in conjunction with local or regional anesthesia, such as an epidural. If sedation is administered the patient need only stay in the clinic for a few minutes until feeling fully recovered.

What are tubular or tuberous breasts and how do they influence breast implants?


This term is applied to an anomaly present in certain patients whose breasts are not only small but also have a special structure. It refers to breasts constrained by a fibrous covering, giving the breast a conical shape, with excessive elevation of the lower furrow, a large areola and with a ruptured aspect, having insufficient skin in the medial area of the breast. In the majority of cases it is possible to obtain excellent results using breast implants and other surgical applications on the gland. Our Specialists have wide experience in the correction of breast anomalies, and thus they know these cases have a higher risk of capsular contracture or hardening and displacement of breast implants, as well as requiring a new surgical procedure after some months to improve the result.

What type of anesthetic is most suitable in otoplasty?

Our anesthetists apply an associated sedative to relax the patient giving them an agreeable sensation of comfort, at the same time allowing for easy procedure of the treatment. Why are there different areas of treatment for the eyebrows, face and neck?


Although the patient perceives each external sign of aging as a separate issue, a profound and functional anatomical knowledge of the signs of aging leads to surgical procedures based on the application of scientific concepts. Several groups of treatment exist that are applied to different areas achieving various simultaneous effects. Problems appearing on the surface cannot be separated individually, because the situation on the inside causing the problem is more complex. Specialists speak of the upper 2/3 of the face, an area including the forehead, orbital region, eyebrows and cheeks, and of the lower 2/3 of the face, affecting the cheeks, jawline and neck.

Are stretching or simple lifting techniques obsolete?


Yes. Nowadays Cosmetic Surgery has reached a high degree of scientific knowledge and corresponding results. Simple skin stretching techniques produce completely artificial results with a "stretched" face, lasting only a relatively short time. Frequently they are associated with disagreeable consequences or very poor quality scars. No. Statements like these are indefensible contradictions. Lifting is a surgical treatment that is irreplaceable when reconstructing the features of the face that have suffered the effects of aging. Although it is true it is not the only option, the same effects cannot be obtained with any other procedure. It must not be forgotten that the passing of the years is shown primarily in the sagging of the deep tissue and muscles in the face, and that the skin is often a minor factor.

Can I lose my rights and my money during the reservation procedure?

Even if you have not been through pregnancy, if the patient has been obese and then lost substantial weight, the characteristic result is an excess of skin in the abdominal region, for which the abdominoplasty is an excellent solution.

Is the postoperative stage painful?


Our Medical Team performs treatments designed to cause patients the least amount of pain. In the postoperative stage effective analgesics are administered that calm the pain, especially during the first week after the operation when there is the most inflammation. In general, the operations that we perform are not specially painful and the postoperative period evolves usually without causing the patient any difficulties. If any postoperative is remarkable due to pain, it is the one after breast augmentation, as a consequence of implant placement under tight skin, mainly concentrated the first four days after surgery.

What is included in the estimate for a treatment?


The estimates for our treatments are created on the basis of protocol determined by our Medical Team. An estimate includes all necessary costs to fulfil the provisions established by the Specialists, such as the team, the preoperative tests, hospital requirements and implants or girdles. Can other procedures also be performed in the same ears operation?


This is quite a normal procedure and is much requested by our patients. There is no reason not to perform a rhinoplasty or breast augmentation together in the same operation with an otoplasty, to give an example.

How long do scars take to heal?


All scars pass through a phase known to Specialists as the maturing stage, during which time they are active and can look somewhat reddened and can itch. The process is variable in each patient, between 6 and 12 months as an estimation, but, with time, the scars become progressively more normal until they reach their best aspect.

Do I have to pay before meeting the Specialists?

Insertion channel of breast implants


Implants can be inserted through the areola, through the submammary fold under the breast, under the armpit and other methods mentioned purely as anecdotes (the tummy button, that requires using a saline implant giving deficient results) or by means of an incision made for a mastopexy or breast elevation. It is a consensus among Specialists that the recommended insertion way should be through the areola since it produces the best quality scar, does not alter sensitivity, offers the lesser rate of complications and superior cosmetic results of the placement for the implants, is perfectly hidden and allows for any signs to be totally eliminated with micropigmentation. If the areola is very small then it is recommended insertion via the fold under the breast. Under the armpit is not recommended because of high rate of total loss of sensitivity on the nipples, the high rate of complications like bleeding or malpositioning, the scars are totally visible when wearing sleeveless clothes in summer, and also because the implants tend to sit rather unnaturally high and move when contracting a muscle. The operation is essentially the same in any case. No special technique is involved if using any of a variety of approaches. The use of an endoscope is completely unnecessary in breast enlargement and is only a marketing complement used for commercial purposes in certain centres.

Must I have check ups?


As with any medical or surgical treatment. We apply a meticulous on premise and online protocol in terms of dressings and early and subsequent check ups for our patients. These are more frequent during the first month and then become more staggered over a period of between 6 to 12 months. This period to be prolonged if required.

Are breast implants safe?


One of the biggest fears most common in women who are considering undergoing Cosmetic Surgery on their breasts is the safety of the breast implants. This natural fear is greatly increased by the different information circulating around that is often confusing, inexact, contradictory or quite simply false. The use of breast implants to enhance the shape of the breast began at the beginning of the 70´s and developed importantly in the 80´s in the USA before arriving to Europe during that decade.

What can be done if I want to have a liposuction and there is leftover skin?

What difference is there between liposuction and liposculpture, lipoplasty, etc.?


None. All those terms and many others describe the same liposuction procedure. Liposculpture and others do not consist in anything newer or more special. They do not exist in scientific literature. What means of approach is best to the lower eyelids?

Is the healing really so swift in the eyelids?


The skin in the eyelid region is the area that heals the quickest in the whole body. In only a few days the first internal threads are eliminated. The inflammation lasts rather longer but only considerably so during the first few weeks.

Does liposuction correct excess skin and flaccidity?


No. Liposuction is a technique designed on the fatty tissue and not for the skin. However one of the effects of liposuction is the contraction of the skin to eliminate the excess produced after having removed the inside volume of fat. If beforehand the patient shows a degree of flaccidity and excess skin, postoperatively will have the same degree and the postsurgical correction may not be total, depending on different factors in relation to the quality of the skin.

What documents should the patient receive on finalising treatment?

Will sensitivity be retained at the areola after breast elevation or reduction?


With the majority of techniques it is possible to retain excellent sensitivity of the areola and nipple. Can the vision be improved?

What are the bags on the eyelids?


In the space between the eyeball and the bone that surrounds it there are some bags of fat that occupy this free space. With the passing of the years and aggravated by certain hereditary factors, or from those that have been acquired such as smoking or thyroid problems, these adipose packages grow causing hypertrophy. The increase in size of these bags makes them too big to occupy the original space and they begin to gain prominence, extending over the outer limits of the socket, making them very visible as bags under the skin. There are two bags under the upper eyelids, inner and middle and 3 bags under the lower eyelids, inner, middle and external. The treatment consists in eliminating the excess that overlaps the socket and does not affect the normal function of the whole at all.
